Please note that I ask the questions I do on the following form for planning purposes, to help you arrange special arrangements for large groups, and to start planning for some special gifts that I bring for attendees. (It’s not to push or be invasive).

Hosts of large groups (over 10) are required to pay service fees as applicable to event spaces. Susan T Ouellete will NOT be responsible for any event space fees unless otherwise negotiated for an additional fee.

Please do not proceed if there are malignant / maleficient spirits involved - this is not an area that I am specialized in and would prefer to not work with overall.
